Conversations in Chaos

One morning in February, a group of 20 medical school faculty members sat inside the South Asian exhibit of a local art museum. The 11th-century sandstone sculpture they contemplated depicted the deity Vishnu, who in Hindu tradition periodically descends from the heavens in times of chaos to restore cosmic balance. The observers were prompted to consider what “chaos” looked like to them and what other feelings the word brought forth.

Analyzing Annals

Glucagon-Like Peptide-1 Receptor Agonists and Risk for Gastroesophageal Reflux Disease in Patients With Type 2 Diabetes. A Population-Based Cohort Study

Glucagon-like peptide-1 receptor agonists (GLP-1RAs), which are used to treat type 2 diabetes and obesity, are associated with delayed gastric emptying, which is a risk factor for gastroesophageal reflux disease (GERD). However, evidence linking these drugs to GERD is limited. This cohort study estimated the effect of GLP-1RAs versus sodium–glucose cotransporter-2 inhibitors on risk for GERD and its complications among patients with type 2 diabetes.

Assessment of Shared Decision-making Tool for Eliciting Informed Goals of Care in the Hospitalized Elderly: A Randomized Trial (ASKmeGOC Study)

Up to 75% of patients lack decision-making capacity at the time of their serious illness, with less than 25% having communicated their treatment preferences to their HCP. As a result, many patients receive unwanted and unwarranted medical and surgical interventions. This omission may result in goal-discordant, low-value care, and unnecessary healthcare expenditures in one in eight patients who die in hospital.
